J.R. Martin, MS, MSS, PA‑C, is the founder and lead provider of Alive & Well Direct Primary Care. He has been a certified physician assistant for more than twenty years, inspired by his father—a rural Utah physician who made house calls. After earning two master’s degrees in Health Promotion and Medical Science from Midwestern University, J.R. saw the healthcare system become increasingly rushed and impersonal, so he created Alive & Well to restore personalized, affordable care delivered by phone, video, or in‑person visits to patients’ homes and workplaces.

At Alive & Well, J.R. leads a small team that offers 24/7 telemedicine, on‑site visits, and house calls, emphasizing preventive health, patient education, and cost‑saving solutions. He draws on experience in pediatrics, orthopedic surgery, and nine years in emergency medicine to provide comprehensive family‑medicine services while maintaining the direct‑primary‑care philosophy of building long‑term relationships and eliminating insurance‑driven barriers.
