CLIFFORD GAZDA

Dr. Clifford Gazda was born and raised in Chicago, IL. He earned a BA in neuropsychology from Northwestern University and conducted research on sleep and circadian rhythms at Rush University. He later completed an MA in Biological Sciences at Loyola University Chicago before attending the University of Illinois College of Medicine, where he received pre‑clinical honors in cell and tissue biology, immunology/microbiology, and molecular medicine/genetics. Dr. Gazda completed his residency in Family Medicine at HonorHealth (formerly Scottsdale Healthcare) in Arizona, where he was recognized as Outstanding Second‑Year Resident and received faculty development training.

As a physician at Connected Family Medicine, Dr. Gazda embraces the Direct Primary Care (DPC) model, emphasizing personalized, relationship‑based care without the constraints of insurance billing. He believes the best care comes from shared decision‑making, preventive focus, and ample time with patients. Outside of medicine, Dr. Gazda enjoys time with family and friends, kickboxing, running, and golf.
