FARAH MYERS

Farah Myers is a Family Nurse Practitioner and certified Diplomate with the American College of Lifestyle Medicine at Cool Creek Family Health in Carmel, Indiana. She founded the practice to deliver Direct Primary Care, describing it as “Netflix for health care,” and offers patients a membership model that provides unlimited primary care services without copays.

She emphasizes personalized, relationship‑based care, offering same‑day appointments, extended visit times, and direct access via phone, secure text, video chat, and email. Her focus on lifestyle medicine, preventive care, and affordable lab testing reflects her commitment to improving community health.

Prior to opening Cool Creek, Farah practiced at IU Health, where she observed fragmented care and sought a model that fosters continuity. She holds an FNP‑C credential, a DipACLM certification, and graduated from Frontier Nursing University (and holds a degree from DePaul University). She is dedicated to empowering patients to take charge of their health.
