Dr. Trevin Cardon is a Fort Collins native, having moved here when he was only 5 years old. He attended Brigham Young University for his undergraduate studies and earned a Bachelor’s degree in Spanish Translation with a minor in Chemistry. Dr. Cardon attended the Arizona College of Osteopathic Medicine in Glendale, AZ for medical school and served as a teaching and research fellow in the osteopathic manipulative medicine department. He completed his residency in 2021 at the Fort Collins Family Medicine Residency, where he served as chief resident. After graduation, Dr. Cardon joined the residency faculty while also growing his direct primary care practice, Hometown Family Health, located in Fort Collins. Dr. Cardon is passionate about family medicine and believes in building strong, lasting relationships with patients. He focuses on lifestyle medicine, sports medicine, and osteopathic manipulative medicine, and is dedicated to innovative, patient‑centered direct primary care models.
